Current concepts in microsurgical soft tissue reconstruction of lower extremity trauma in a single-vessel extremity

چکیده
Advancements in microsurgical techniques have allowed for the salvage of extremities that would otherwise be amputated. Despite these improvements, mangling injuries lower extremity with concomitant open fracture continue to present a challenge reconstructive surgeons. These are further complicated by circumstances which single vessel is perfusing foot after trauma (Gustilo 3C injuries). Although may remain perfused this instance, free flap coverage fractures vascular injury requires careful planning maintain viable foot. The surgeon must adapt surgical plans based on both preoperative and intraoperative findings. In review, we discuss current concepts performing reconstruction single-vessel extremities.

متن کاملEarly Reconstructions of Complex Lower Extremity Battlefield Soft Tissue Wounds
BACKGROUND Severe lower extremity trauma as a devastating combat related injury is on the rise and this presents reconstructive surgeons with significant challenges to reach optimal cosmetic and functional outcomes. This study assessed early reconstructions of complex lower extremity battlefield soft tissue wounds. METHODS This was a prospective case series study of battled field injur...
متن کاملAbstract: Soft Tissue Reconstruction and Salvage of Infected Fixation Hardware in Lower Extremity Trauma
BACKGROUND Tibial fracture management may be complicated by infection of internal fixation hardware (iIFH) resulting in increased morbidity and amputation rate. When iIFH removal is not possible, salvage of the lower extremity is attempted through debridement, antibiotics, and vascularized soft tissue coverage. This study investigates lower extremity salvage with retention of iIFH. METHODS De...

PSRC Abstract Supplement P oter P rofs RESULTS: 358 breasts underwent reconstruction before protocol implementation and 135 afterwards. Patients were similar in terms of demographics and surgical characteristics. There was a significantly reduced incidence of clinically relevant infection after protocol implementation (10.1% to 2.9%, p= 0.009). Logistic regression analysis confirmed that the pr...
